The South Devon Railway have produced a carriage fault reporting book based on over 40 years of running trains in a heritage environment.
The books are multipart NCR with intergral seperator page.
Each page is perforated along the top for removal and drilled on the lefthand side for filing.

The design assumes the top copy (white) is kept as the audit trail and signed off when repairs are effected.
There are two copy pages to allow for body repair requests to be seperated from running gear repairs if required.

Every report is serial numbered so the originating book can be found for the repair update and the books can be audited if required.
